Ya Hayyu Ya Qayyum Birahmatika Astagheeth Dua

يَاحَيُّ، يَا قَيُّومُ، بِرَحْمَتِكَ أَسْتَغِيثُ، أَصْلِحْ لِي شَأْنِي كُلَّهُ، وَلَا تَكِلْنِي إِلَى نَفْسِي طَرْفَةَ عَيْنٍ أبدا

Yaa Hayyu yaa Qayyoom birahmatika astagheeth ‘aslih lee sha’nee kullahu wa laa takilnee ‘ilaa nafsee tarfata ‘aynin abada.

Translation:
O the Ever-Lasting, O the Sustainer and Protector of all that exists. I seek aid through Your Mercy; correct for me all my matters and do not entrust me to my own self even for the blink of an eye, ever.

Derivation
Anas bin Malik reported:
“Whenever a matter would distress him, the Prophet (ﷺ) would say: *‘O Living, O Self-Sustaining Sustainer! In Your Mercy do I seek relief (Yā Ḥayyu yā Qayyūm, bi-raḥmatika astaghīth).”
